What the appointment form asks for
The public form requests a name, mobile number, department of interest, preferred time and optional preferred date. It is intended for non-urgent appointment requests only.
Where a request may go
If the ERP database is installed, the form attempts to save an appointment request and the contact details needed to respond. The package can also email the request using the hosting server’s PHP mail service. The exact delivery and retention process depends on how the hospital configures its hosting and email.
Information to keep out of the public form
Do not submit reports, detailed medical history, passwords, OTPs, payment PINs or urgent symptoms through the public appointment form. Contact the hospital directly for instructions about sending records through an approved channel.
External services and stock images
Google Fonts, embedded Google Maps and remote image providers may receive connection metadata from visitors’ browsers when those elements load. Replace third-party map or media elements if the hospital’s privacy policy requires it.
Hospital approval needed
Before going live, the hospital must set and publish the contact for privacy questions, lawful basis, access rules, retention period, incident process and any applicable local legal notices. This template is not legal advice.
